JOB SUMMARY: Assists patients in maintaining or improving their ability to manage their everyday physical, mental, and psychosocial needs.
ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:
· Assesses patients admitted for discharge planning needs, coordinates discharge planning activities, and follows through with interventions.
· Provides progress report to physicians referring patients for social services.
· Makes arrangements for obtaining needed adaptive equipment, clothing, personal items, dental care, podiatric care, eye care, and hearing services.
· Facilitates Mental Health transfers, alcohol/drug abuse treatment transfers, and abuse cases.
· Organizes and coordinates multidisciplinary discharge planning meetings for all Skilled patients as well as any patient needing additional services.
· Collaborates with nursing on Crisis Intervention cases, provide training for staff, and be available for call-in consult as necessary.
· Finds options that meet the physical and emotional needs of each outpatient including grief counseling.
· Screens all admissions; keep families informed of patient’s progress and discharge planning when necessary.
· Coordinates between patient and family any medical needs, social needs, and discharge planning. Assists in arranging financial assistance through the appropriate agencies.
· Coordinates patient’s transitions from inpatient to outpatient rehabilitation, home health care programs or other facilities.
· Aides in the planning discharge of patients to other facilities; and is also responsible for the necessary documents for that level of care (ID Screen, Senior Care Options, etc.)
· Coordinates with other agencies for preadmission certification/complete PASRR.

$25.00 - $35.00 per hour depending on experience
QUALIFICATIONS:
· Minimum of a BSW, RN, or degree related human services field including sociology, special education, rehabilitation counseling, and psychology
· Prior care coordinator experience in medical field preferred
· Dependent Adult & Child Abuse Mandatory Reporter Training
