Social Worker

Position Summary

We are seeking a compassionate and experienced Medical Social Worker to support patients and families in the Denver area. This role focuses on psychosocial assessments, care coordination, community resource planning, and emotional support for clients navigating health-related challenges.

Key Responsibilities

• Conduct psychosocial assessments for patients and families
• Develop and implement care plans based on individual needs
• Provide counseling, emotional support, and crisis intervention
• Assist with long-term planning and access to community resources
• Coordinate with physicians, nurses, therapists, caregivers, and family members
• Support discharge planning and transitions of care
• Maintain timely and accurate documentation
• Ensure compliance with company standards and applicable regulations

Qualifications

• Master’s Degree in Social Work (MSW) preferred
• Current Colorado Social Work license preferred (LCSW / SWC, if applicable)
• Previous experience in home health, hospice, hospital, or healthcare setting preferred
• Strong communication and interpersonal skills
• Ability to work independently and manage caseload effectively
• Knowledge of community resources and patient advocacy
• Reliable transportation preferred
