Social Worker - Part Time

DaySpring Hospice is seeking a Social Worker to work 3 days per week. This position will coordinate support services for our hospice program. Directs social work component to include developing and implementing psychosocial plan of care for patients and families. Visits and counsels with patients and/or families as needed regarding pastoral care services. Coordinates pastoral referrals and monitors spiritual needs of patients.

Performance Duties and Responsibilities

• Participates in patient care conferences and meetings.
• Develops ongoing psychosocial plan of care for patients and families admitted to hospice.
• Treats patients, physicians and fellow employees with care, courtesy and respect.
• Interprets and assess social and emotional factors related to patient illness, need for care, response to treatment and adjustment to care.
• Assists with discharge plans for hospice patients. Maintains current information on community resources, insurance, ect.
• Makes initial psychosocial evaluation to each patient upon admit.
• Develops and implements individualized bereavement plan of care for families.
• Visits and counsels with families in the bereavement process.
• Assists families with arrangements for funeral when necessary

Masters Degree in Social Work or LBSW

Licensed to practice in the state of Alabama

2-3 years recent health care social work experience, hospice experience preferred
