What You’ll Do:

• Conduct comprehensive psychosocial assessments and develop individualized care plans.
• Provide counseling, emotional support, and crisis intervention for patients and families.
• Connect patients and families with community resources and support services.
• Collaborate with an interdisciplinary team to ensure holistic, patient-centered care.
• Maintain timely, accurate documentation in compliance with regulatory and organizational standards.

What You'll Bring:

• Degree in Social Work
• Current state licensure in social work
• Strong interpersonal, communication, and organizational skills
• Ability to work independently while contributing to a collaborative team environment
• A compassionate, patient-focused approach with the ability to navigate sensitive situations professionally
