Job Description - Speech Language Pathologist (SLP)

JOB TITLE: Speech Language Pathologist (SLP)


STATUS: Part-time


DEPARTMENT NAME: Clinical


REPORTS TO: Clinical Director and Administrator of Home Care and Home Health


COMPENSATION: $100 per visit




EverHome HealthCare is a well-established and highly respected Home Health and Home Care agency serving the Snohomish, King, and Pierce counties. We are a doctor-owned company with an expert team to support you. We are looking for an experienced Speech Language Pathologist (SLP) to join our Clinical team. We provide in-home visits to our clients to assess, treat, and improve language skills.



Duties, Responsibilities, & Accountabilities Including but Not Limited To:


The following duties and responsibilities reflect the essential functions of this job but do not restrict the tasks that may be assigned. The Director may assign or reassign duties and responsibilities to this job at any time due to reasonable accommodation or other reasons.



  •  Develops, implements, and evaluates comprehensive speech pathology plan of treatment/plan of care in collaboration with physician and client or caregiver following Agency guidelines.

  •  Provides therapeutic intervention and training to improve functional communication ability following physician orders.

  • Recommends equipment to improve function and independence in communication.

  • Instructs and assists staff and family members in certain phases of speech pathology.

  • Observes, records, and reports on the client's reaction to treatment and any changes in their condition.

  • Maintains and documents communication with physicians and other health care professionals to assure continuity of care.

  • Submits required documentation in an accurate and timely manner following Agency policies and procedures.

  • Communicates and coordinates work schedules with the schedule, to ensure continuity of client care.



Requirements:



  • To be eligible you must have an active Speech/Language Pathologist license in the state of Washington.

  • Certificate of Clinical Competence (CCC) from ASHA is a plus

  • Minimum of one (1) year of speech therapy experience required.

  • Current CPR/BLS

  • Reliable transportation, current Driver’s License, and proof of auto insurance
