Job Title: CFSS Support Worker - Community First Services and Supports (CFSS)

Job Summary

The primary purpose of this position is to work directly with individuals receiving Community First Services and Supports (CFSS) in the implementation of the written service delivery plan, documenting progress and response to program services, and ensuring the safety and well-being of the participant. This position is responsible to the assigned Executive Officer and the Supervising Professional. 

Role Specifics

Shift: AM 27.5 hours/week

• Monday 10:30am-4pm
• Tuesday 10:30am-4pm
• Wednesday 10:30am-4pm
• Thursday 10:30am-4pm
• Friday 10:30am-4pm

Location: St. Paul, MN 

Rate of Pay: $17.00/ hour

*Individual lives with a roommate and has a dog. Would like to move into his own home without his roommate

Essential Duties & Responsibilities

• Assists participant(s) with daily routine including but not limited to activities of daily living, instrumental activities of daily living, health-related procedures, and tasks as specified in the participant’s service delivery plan. 
• Notifies assigned Executive Officer and/or Supervising Professional of service-related or employment-related emergencies or incidents. 
• Establishes and maintains positive working relations and open communication with participant and participant’s representative. 
• Maintains confidentiality of all information regarding the participant. 
• Maintains all employment standards, required training, and licenses.
• Attends and participates in education programs designed to increase knowledge and experience in the field as assigned. 
• Follows and completes cares outlined in the services delivery plan and charts accurately, turning in to Supervising Professional as directed.

Working Conditions

• May be subject to hostile and emotionally upset participant(s) staff, family members, visitors, and personnel from other agencies.
• May be exposed to a variety of conditions, infectious diseases, odors, dust, etc. throughout the working hours.
• Is subject to flexible hours, including split shifts or varied days and times including mornings, afternoons, and evenings, holidays, and weekends depending on coverage needs.
• May be exposed to animals, including but not limited to dogs, cats, pet rodents, birds or pet reptiles.

Physical Demands

• Sits, stands, walks, bends, and moves intermittently during working hours.
• Lifting/transferring may be required based on participant’s need.
• Ability to move intermittently throughout the workday.
• Possess good health and demonstrate emotional stability.
