Job Description - Dartmouth Health Movement Disorders Coordinator Neurology Part-Time

The Coordinator for the Dartmouth Health Movement Disorders Division will be responsible for coordinating various aspects of the Parkinson’s and Huntington’s centers of excellence. This will include working closely with the Co-Directors to develop and implement plans to fulfill the mission of the Center of Excellence, including patient education and allied health professional outreach throughout northern New England, promoting quality improvement and research projects that improve the comprehensive care for patients with Parkinson’s disease. Essential to this position, the Coordinator must maintain excellent relationships with patients, families, physicians, stakeholders and will serve as the primary liaison for the DH-PF-COE, communicating by email and video conferencing with patients and patient support groups, allied health providers, and Parkinson Foundation personnel.

  1. Daily
  2. Work with Social Worker, Neurology Secretary to reply to emails and phone messages left to the Center
  3. Keep the Center patient email database up to date
  4. Function as a liaison between Dartmouth-Hitchcock COE and the Parkinson’s Foundation
  5. Function as a liaison between the Dartmouth-Hitchcock COE and the Parkinson’s Community at large (Upper Valley Parkinson’s, Exercise classes, and Key Stakeholders)
  6. Assist people with PD and families in navigating a disease treatment plan, promoting empowerment and self-advocacy
  7. Weekly
  8. (Assess clinic patient education materials weekly and restock as appropriate)
  9. Monthly
  10. Parkinson’s Foundation Center of Excellence Coordinator
  11. Help Social Worker maintain an up-to-date list of area support groups, individual email database, as well as community activity programs
  12. Coordinate with Center Co-Directors in planning of monthly multidisciplinary and DBS team meetings, work with Neurology secretary to distribute an agenda to key stakeholders.
  13. Write and distribute a monthly Parkinson’s newsletter by email to patient distribution list
  14. Huntington’s Disease Center of Excellence Coordinator
  15. Compose and send out HD newsletter every other month
  16. Monitor email for HD patient issues and especially for new patients reaching out for appointments using that email
  17. work with local HDSA chapter
  18. work with HD specialist Physical Therapist on coordination of care for HD clinic and PT appointments.
  19. Periodically
  20. Work with Marketing semiannually to maintain the DH-PF-COE web site
  21. May be asked to participate or direct projects that involve the PF and/or our COE, such as quality improvement projects
  22. Be responsible for the execution and planning of a minimum of one, but up to three education conferences/symposiums, ensuring project completion on time and within budget (this includes securing the venue, securing the speakers, planning the brochure/disturbing along with marketing, working with marketing to advertise, ordering “swag” for patients/speakers, and coordinating the event)
  23. Secure speakers for the PD-101 series through the Aging Resource Center
  24. Participate in in new or re-designation evaluations as a peer reviewer as requested
  25. Help establish and maintain a Parkinson’s mentor/mentee program for patients
  26. Participate as a mentor or mentee in the Mentoring and Networking Program
  27. Annually
  28. Represent DH-PF COE at outreach activities including Parkinson’s Foundation Moving Day (this includes registering the Dartmouth-Hitchcock team, setting up a DH table at the event with appropriate brochures/information and “swag” for patients, coordinating with the PD foundation on this event)
  29. April is Parkinson’s Awareness Month. Yearly our center does something special in recognition for this month. The coordinator will be responsible for this.
  30. Work with Co-Directors to develop and implement quality measures
  31. Prepare an annual report to the Parkinson Foundation as required
  32. Attend the Parkinson’s Foundation Leadership meeting and Coordinator’s day every December (costs paid for by the Parkinson’s Foundation)
  33. Attend the Foundation’s annual Virtual Coordinator’s Day
  34. Work with DH finance department to account for the PD COE budget and submit completed budget documents to the Foundation
  35. Assist in re-application for PCE designation every 5 years
  • Must be capable of dealing with periodic cyclical workload pressures and varying levels of responsibilities.
  • Must be able to carry through projects to completion
  • Must be a Registered Nurse or Masters level healthcare professional (PT/OT/SLP/MSW/APRN/ etc.)
  • Ability to work independently and as part of a team
  • Bachelors of Science or Master’s degree preferred
  • Knowledge of Parkinson’s Disease is a plus
