Healthcare Care Coordinator (West/ Up to $3000) #HCGE

Responsibilities

• Build and maintain positive relationships with patients, caregivers, and healthcare team members.
• Support patient care coordination and follow-up activities.
• Use IT systems to track, document, and monitor patient care and clinical indicators.
• Conduct basic health screenings and measurements.
• Promote preventive health screenings and vaccinations in line with national health initiatives.
• Provide basic health education on healthy lifestyles, disease prevention, and self-monitoring of health conditions.
• Guide patients on the use of home health monitoring devices such as blood pressure monitors, glucose meters, and weighing scales.
• Assist patients in accessing healthcare and community resources to improve their overall well-being.
• Leverage technology to enhance patient engagement, health literacy, and access to care.
• Other ad hoc duties assigned by the supervisor

Requirements

• NITEC, Diploma, O-Level, or A-Level qualification.
• Good verbal and written communication skills.
• Pleasant personality with strong interpersonal skills.
• Positive attitude, self-motivated, and a good team player.
• Interested in patient care and community health.
• Proficient in Microsoft Office applications (Word, Excel, and PowerPoint).
• Comfortable using IT systems and digital tools.
