Locations available: West side polyclinics OR Queensway

Job Responsibilities:

• Attend to telephone and email enquiries for Medical Social Service.

• Provide administrative support to the Medical Social Worker on referrals, accompany social worker on field visits and assist in interpretation if required.

• Gather information and conduct preliminary investigation on patients' needs to facilitate intervention by Medical Social Worker. 

• Process application and ensure submission are in compliance.

• Maintaining the work statistics and compile the data for submission to Ministry of Health.

• Handle external correspondences such as MP letters and CDC letters.

Requirements:

• Relevant education qualification in Social Work or equivalent
• At least 1 year of working experience in community health
• Willing to travel between polyclinics occasionally and do ad-hoc home visitations together with Medical Social Worker
