Assistant / Care Coordinator (Health Promotion) - Ref:HC

Job Scope:

• Ensure completion of screening, assessment, anticipatory guidance and psychoeducation for mother-child dyads according to milestones
• Assist in care management and cross-referrals to specialists through dedicated protocols
• Maintain a record of mother-child dyads/parents recruited in the program in timely basis
• Work with other departments to assist with arrangement of appointments
• Work closely with research assistant in data collection and survey
• Ad hoc duties as assigned

Job Requirements:

• Diploma in Health Promotion / Health Sciences or equivalent
• Competent in MS Office
• Good communication and interpersonal skills
• Candidate with experience in healthcare industry is at advantage
