As a Care Coordinator (Healthcare), you will be responsible for the following duties:

• Collaborate with the multidisciplinary care team to deliver holistic, patient-centred care.
• Empower patients and caregivers to manage their health through the use of digital health technologies and tools.
• Promote awareness and access to community and healthcare resources to support positive health outcomes.
• Build and maintain strong relationships with patients, caregivers, and internal stakeholders.
• Support preventive health initiatives by encouraging health screening and vaccination uptake.
• Collect and document patients' social determinants of health to support care planning and interventions.
• Coordinate and support patient care activities using IT systems for documentation, tracking, and monitoring clinical indicators.
• Perform basic health assessments and assist in preventive care programmes.
• Provide health education on lifestyle management, self-care, and physical and social well-being.
• Educate patients on the proper use of home health monitoring devices, including blood pressure monitors, glucometers, and weighing scales.
• Advise patients on recommended vaccinations and national health initiatives.
• Perform any other ad hoc duties as assigned.

Requirements:

• Min. Diploma in Health Sciences (Health Management) or other related field.
