Job Description - Care Coordinator (Healthcare / Restructured) #HJO

Responsibilities and Duties:

Carry out discharge plan assessment for inflight cases.
Provide patients and their family members with relevant and timely information and identify/assess their needs when choosing a suitable Community Hospital.
Document the assessment, care plans, and outcomes timely and accurately in the appropriate systems.
Monitor the referral trends and identify service gaps/transfer issues for rectification.
Establish and maintain partnerships between Hospitals and Community Hospital referral teams.

Job Requirements:

Degree in Health Science/Nursing.
A good team player who is able to work efficiently in a multi-disciplinary team
