Case Management Officer (Psychological Medicine) - Ref:HC

Job Scope:

• Responsible for the delivery of Neuropsychological assessments
• Responsible for the arrangement of care plans for Psychogeriatric and Child/Adolescent patients
• Administer brief psychogeriatric assessments and brief child assessment (Continuous Performance Testing) and collect basic socio-demographic information for patients
• Obtain clinical information from patients via use of scales (ADHD, depression, anxiety rating scales) prior to patients’ first visits
• Confirm with patients for their attendances of first visit / follow up appointment
• Triage cases based on their clinical needs
• Perform neuropsychological assessments for patients with cognitive complaints, under the supervision of a Senior Psychologist
• Provide psycho-education for patients and their family members
• Facilitate discharge to step-down care (polyclinics, GP clinics, SLH etc) once diagnosis and care plans are finalized
• Ad hoc duties as assigned

Job Requirements:

• Degree in Nursing, Social Work, Psychology, Allied Health or equivalent
• Good interpersonal and communication skills
• Proficient in using MS office
• Knowledge in geriatric, adolescent/child care and community skills will be an advantage
