Job Description - Endocrinology Clinic - Clinic Nurse / Nurse Educator

Working Hours: 5-day office hours (Monday - Friday)

Employment type: Full-Time, Permanent

Specialist Outpatient Clinic (Endocrine)

Deliver diabetes counseling and education to patients and their families
Ensure regular follow-up for patients with suboptimal glycemic control through various communication channels.
Recommend treatment plans (for example insulin titration), work closely with other HCPs to contributing to the multidisciplinary care team.
Conduct caregiver training sessions as needed to enhance patient support and management.
Engage in evidence-based practices and nursing research endeavors, fostering continuous improvement.
Lead and participate in quality management audits and initiatives.
Conduct specialized staff training sessions, serving as a knowledgeable resource for consultation.

SNB Registered (Full)
Keen and passionate in educator & nursing role
Well-versed in diabetes management / patient education
