ROLE
The Executive, Case Coordinator is responsible for coordinating and managing the delivery of home-based care services to ensure clients receive person-centred, safe and quality care in the community.
The incumbent will conduct comprehensive care assessments, develop and review care plans, coordinate multidisciplinary services, supervise Community Caregivers, and work closely with clients, caregivers and community partners to support ageing-in-place and improve client outcomes.
DUTIES AND RESPONSIBILITIES
1.Home-Based Care Services
A. Client Assessment & Care Planning
· Conduct comprehensive InterRAI Check-Up assessments for new clients and perform periodic reassessments to review clients' care needs and outcomes.
· Develop, implement and review individualised care plans based on clients' functional, psychosocial and environmental needs.
· Identify risks, recommend appropriate interventions and make referrals to internal and external service providers where required.
· Maintain accurate, timely and comprehensive documentation of assessments, care plans, interventions and case notes in the required systems.
B. Case Coordination & Case Management
· Coordinate and monitor the delivery of home-based care services to ensure continuity and quality of care.
· Work closely with Community Caregivers to monitor clients' progress, address emerging issues and ensure care plans are effectively implemented.
· Liaise with multidisciplinary teams, healthcare institutions and community partners to coordinate holistic care and appropriate follow-up services.
C. Community Caregiver Support &Supervision
· Provide guidance, supervision and operational support to Community Caregivers to ensure quality service delivery.
· Monitor caregiver performance, provide feedback and escalate care concerns where appropriate.
· Support caregiver competency development through training, coaching and ongoing engagement.

Requirements criteria:
· Diploma or Bachelor's Degree in Nursing, Social Work, Allied Health or other relevant disciplines.
· Preferably with experience in community care, healthcare, eldercare or case management.
· Competency in conducting InterRAI assessments will be an added advantage.
· Able to remain organised, adaptable and composed while working in a fast-paced, high-pressure environment.
· Strong interpersonal, communication, organisational and problem-solving skills.
· Proficient in Microsoft Office applications and electronic care management systems.
