Home Care Nurse (Wound Management)

One of the leading private healthcare organizations in Singapore, providing home care services (wound management), renowned for its commitment to excellence in patient care is now hiring! This is a full-time on-site role for Staff Nurse, taking care of the West Region (Jurong/Choa Chu Kang/Clementi).

Working hours

• Monday - Friday Office Hours
• Saturday Half day (Twice a month)

Responsibilities

• To conduct home visits to provide assessment and nursing care to clients.
• Providing care for both acute and chronic wounds.
• Initiating appropriate wound management for the patients namely negative pressure wound therapy (NPWT), conventional dressing and application of 2 and 4 layers compression bandage.
• Educate patient and family members on wound healing process and issues.
• Educate and counsel patient and family members on care of stoma and choosing the appropriate stoma appliances.
• Demonstrate stoma care to the primary caregiver.
• Trainings will be provided, on the job training & observation/shadowing

Requirements

• SNB Registered
• Prior experience in Wound Care / Home Care is highly preferred
• Excellent communication skills, and the ability to work collaboratively with other healthcare professionals to manage patient care and promote overall health and wellness
