Duties:
- To uphold nursing standards for patients admitted to homecare as well as care for families and loved ones.
- To coordinate teamwork in total patient care with all members of the homecare team in order to deliver quality care and service to patients and families.
- To work in collaboration with the Advanced Practice Nurse, NC/NM/NE and other nursing staff as appropriate to put in place clinical processes that are conducive to the maintenance and improvement of standards of nursing care in Home Care.
- To ensure accuracy of all documentations related to patient care
- To ensure holistic assessment of patients and their families and caregivers in order to formulate a core plan.
- To work with the rest of multi-discipline team to ensure that the holistic needs of the patients and their caregivers are met.
- To have knowledge of all equipment in use as well as be responsible for the use of equipment for patients in Home Care.
- To be part of the nursing teaching faculty as required and to be part of the process of quality improvement through review of care processes, active participation and/or leadership in projects as required.
- Must be committed to upgrade knowledge and skills in palliative care nursing by attending seminars, in-service and external education.
Requirements:
- Min. Diploma in nursing
- Valid nursing board licence
- Min. 3-8 years of working experience as a nurse in acute inpatient ward setting is required
