Responsibilities

• Speak with patients and their families to understand their emotional, social, and support needs.

• Explain and help patients and families apply for financial assistance, and community support services that they are eligible for.

• Work closely with doctors, nurses, and other healthcare professionals to plan for discharge and smooth transition of care.

• Create and carry out care and support plans together with the healthcare team.

• Keep accurate records, and documentation of interventions, and follow-up care.
