Nurse Counsellor (REF:RT)

• Identify patients suitable for nurse counselling.
• Evaluate patients' social history, current lifestyle, and dietary habits.
• Work closely with GPs to provide individualized care plans.
• Perform eye and foot screenings with educational components.
• Assess patients' mental well-being and refer them to GPs when necessary.
• Assist the administrative team in tracking and monitoring clinical and outcome indicators for the clinics.
• Coordinate with other staff to schedule patients for ancillary services.
• Support the Primary Care Network (PCN) in quality improvement projects aimed at achieving better clinical and process outcomes.
• Other duties assigned by superiors.

Job requirements:

• Advanced Diploma in Nursing and above.
• Min. 4-5 years of relevant experience.
• Registered Nurse.
