Physiotherapist (Home Therapy (West) /Day Rehabilitation Centre)

• Conduct Initial Assessment and using relevant outcome measures

• Design Individualised care Plan and goal settings

• Provide treatment intervention at home setting

• Caregiver training and equipment prescription

• Discharge planning and refer to other services if required

• Amends monthly home therapy meeting

• Do therapy inservice and attend the training organised by the department

• Handle other admin duties

 Job Requirements:

• Degree/Diploma in Physiotherapy recognised by Allied Health Professions Council

• Full registration with Allied Health Professions Council

• Good interpersonal skills

• Independent in conducting assessments and intervention at home setting

• Design the individualised care plan tailored to each patient
