Senior/ Medical Social Worker (Community/ Day Care/ Home Nursing)

Responsibilities:

• Handling social work matters to ensure social care needs for seniors and family members are met holistically.
• Supports planning and implementation of programmes
• Provide psychosocial assessment, intervention and case management to clients
• Develop and implement new community development initiatives
• Network with community stakeholders such as Agency for Integrated Care, Social Service Organisations and Restructured and Community hospitals for effective care integration
• Ensure smooth execution of the Caregiver Training Programme including administering the curriculum and training schedules.

Requirements:

• Degree in Social Work
• Registered with Singapore Association of Social Work
• At least 5 years of related experience
