Work Location: West side polyclinics

Job Responsibilities:

• Deliver safe and effective nursing care for patients with acute and chronic medical conditions.
• Perform patient assessment, triage, treatment and follow-up care.
• Support nurse-led services in chronic disease management.
• Provide health education, preventive care, vaccinations and lifestyle counselling.
• Administer medications and treatments in accordance with clinical protocols.
• Collaborate with physicians and allied health professionals to ensure coordinated patient care.
• Maintain accurate clinical documentation and adhere to nursing and infection control standards.

Requirements:

• At least 2 – 3 years’ Staff Nurse experience, in community/primary healthcare settings will be a strong advantage

• Registered Nurse with Singapore Nursing Board
• BCLS & AED Certified
• Degree or Diploma in Nursing
