Staff Nurse (Home Care)

Broad Functions:

• Patient care management
• Patient coordination
• Medical resource administration
• Provide assessment, planning, implementation and evaluation of client/caregiver quality of care on home visit
• Involved with Home Medical, Home Nursing, and Case Management

Main Responsibilities:

• Assist the nursing team in providing personalised, specialised and cost-effective care
• Monitor and report patients’ conditions and progress to senior members of the nursing team.
• Notify senior members of the nursing team of all unusual observations and/or happenings.
• Assist in the execution of discharge plans of patient care according to the assessed needs, as well as assist in the post-discharge care coordination of patients.
• Follow up on phone reviews and home and/or agency visitations to ensure that services required in the community are in place.
• Perform home visits and assist in educating patients/caregivers on self-care management in the community.
• Assist senior nurses on follow-up phone reviews and home/agency visitations to ensure that services required in the community are in place.
• Educate patients and family members on the patients’ health conditions and help them gain a strong understanding of the holistic care as necessary.
• Arrange appointment with the client and/or caregiver for new referral home visit.
• Conduct and document nursing assessment as appropriate.
• Raise individual care plan based on the identified assessment needs.
• Provide nursing services as required, e.g. change of nasogastric feeding tube, urinary catheter change, dressing change, stoma care, monitoring medication compliance and medication packing, parameters/peripheral blood glucose monitoring, etc.
• Ensure that safety and infection control precautionary measures are practised during home visit.
• Issue receipt and receive payment from the client/caregiver at the end of each visit.
• Hand over cash payment collections to the Finance department after home visit.
• Conduct first assessment as appropriate to identify required needs, e.g. referral for Senior Mobility Fund (SMF), Community Services or Financial Assistance.
• Orientate, teach and guide new nurses on the job training (OJT) for home visits.
• Participate and share knowledge and skills in peer education.
• Safety checks on all equipment used for home care.
• Contribute to administrative work as directed by the Senior Nurse Manager or Nurse Manager.
• Accept additional responsibilities as directed by the Senior Nurse Manager or Nurse Manager.
• Assist the Senior Nurse Manager or Nurse Manager in handling complaints and initiating service recovery.
• Assist the Senior Nurse Manager or Nurse Manager to audit and or appraise the performance of new/junior nurses.
• Assist in supervising junior nurses as and when required.
• Initiate and maintain professional growth through Continuing Professional Education (CPE).

Job Requirements:

• Diploma/Advance Diploma or Degree in Nursing
• Registered with Singapore Nursing Board
• Basic Care Life Support (BCLS) Certification
• Minimum 3 years of relevant work experience. Candidates with more years of experience may be considered for senior position
• Good communication skills; able to buil rapport easily
• Confident and poise in delivery of care
• Team-player
• Able to educate and advise clients and caregivers
• Willing to travel to clients' home
