Staff Nurse (Homecare)

Job Description

• Assesses, plans, implements and evaluates care for patient. Provide a safe and personalised nursing care for patients.
• Communicates and discuss patients’ treatment with Medical team and other health-care providers in the smooth delivery of patient care and services.
• Carries out treatments and procedures as prescribed by physicians in accordance to the standard of nursing practice to meet patients’ physical and emotional needs.
• Documents care given and monitoring outcomes on the established charts and forms.
• Coordinates all patient-related activities for continuity of care. Follow up to ensure continuity of patient care and treatment plans.
• Dispensing of medicines to patients. Responsible for safe-guarding of patients. Dispense medicines according to the 5 rights of medication administration.
• Displays compassion, a positive and caring attitude towards patients, internal and external clients.
• Provides patient/ family information and education on disease prevention, process and rehabilitation.
• Adhere to infection control practices.
• Maintains a clean and safe working environment.
• Respects and protects patients’ rights and confidentiality.

Job Requirements

• Plan & coordinate, to provide holistic, safe and personalized quality care for patients.
• Provide nursing procedures for patients in accordance to standard of nursing practice.
• Provide support to patients. Lead discussions with patients about their wishes and care needs to provide holistic care.
• Anticipate and monitor patients’ conditions, trigger prompt escalation and management to Home medical team.
• State registered Nurse with full registration with Singapore Nursing Board.
• BCLS and AED certificate.
