Job Description
Advance Care Planning (ACP) Facilitator
As an integral part of the Care and Integration Department, your main responsibility will encompass:
- Facilitate and conduct ACP discussion with patients and next of kin
- Process and manage referrals for ACP discussions from inpatient, outpatient and community settings
- Coordinate among patients, families and healthcare team for ACP facilitation/follow up
- Make referrals to community services related to ACP matters
- Provide support to ACP Coordinator in ACP referrals and enrolment and data input as needed
- Conduct talks on ACP and participate in public awareness events
- Assist in audits, research, surveys or quality improvement projects related to ACP
- Preparation of reports and statistics as required
Requirements
- Professional qualification in Social Work, Nursing, Allied Health or any relevant clinical disciplines
- 3 years of experience as an Allied Health Professional
- Experience in a Hospital setting preferred
- Experience/Certification in Advance Care Planning Facilitation (General and Preferred Plan of care) is an advantage
