Job Description - Assistant Manager, Care Coordination Office (Clinical Services)

Job Responsibilities:

  1. Lead the National Dental Centre Singapore (NDCS) Care Coordination Office team to engage the CHAS dental clinics to expand our current primary care network to better support our Singapore population.
  2. Promote strong partnerships with the primary dental care through the organization of continuing professional education events to enhance awareness on the appropriateness of referral.
  3. Review the effectiveness of care transition processes and guidelines to increase right-siting of patients to primary care partners.
  4. Establish suitable patient care coordination and communication channels between primary care professionals and NDCS.
  5. Develop strategy to onboard Polyclinics and General Dental Practitioners for right-siting of NDCS patients and establish patient care coordination and flow between the primary care.
  6. Develop feedback mechanisms with patients for regular review to ensure good levels of satisfaction of services.
  7. Review and expand the various types of dental procedures which can be offered to patients outside of NDCS.

 

Requirements:

  • Degree from a recognised university with at least 5 years of relevant working experience, preferably with knowledge and familiarity of the healthcare sector.
  • High degree of attention to detail.
  • Strong organisational, multi-tasking, decision-making and problem-solving abilities.
  • Strong written and verbal communication and presentation skills
  • Ability to work independently as well as a team and develop interpersonal relationships with internal and external stakeholders
  • Passion, integrity, positive attitude and self-directed
