Care Coordinator / Assistant (Fast Shortlisting) (REF:RT)

• Promote care through active engagement with patients.
• Plan and conduct activities, provide information on any relevant preventive health and diabetes screenings to support health initiatives.
• Provide clinical support which includes measuring patient’s parameters, perform screening tests and educating patients of devices and consumables.
• Provide education on basic lifestyle management, various home monitoring measurements such as blood pressure and heart rate monitoring.
• Make appropriate referrals, assist patients to schedule follow‐up appointments.
• Liaise with external vendors.
• Coordinate, schedule and track appointments.
• Perform any other duties that may be assigned.

Job Requirements:

• Degree in Nursing / Psychology / Social Work / Counselling.
• Relevant experience will be an added advantage.
