As a Case Management Executive, you will be responsible for the following duties:

• Assess the requirements of seniors who live in communities using tools like the Community Screener, EBAS (for depression), and AMT. 
• Examine the care plan with the appropriate parties.
• Encourage the prompt and economical referral and provision of services to seniors 
• Organize care in collaboration with company services and outside providers.
• Engage in case management conversations both inside organisation and with outside service providers. 
• Create a resource map with services and other points of contact for seniors residing in the center boundary.
• Active aging programs delivered via satellite and on-site Groupwork Outreach events
• Keep records in order for audits and prepare data for submission to MOH and AIC
• Assisting volunteer management executives 
• Verify adherence to the policies on finance and human resources. 
• Other Ad-hoc assigned

Requirements:

• Diploma or Degree holder preferably in community and social services (senior services) or Psychology or Counselling or healthcare services.
• Experience in the social service sector will be an advantage.
• Passionate to work and interact with seniors in the community.
