Case Management Officer (Medical Affairs Non Clinical Nursing) - Ref:HC

Job Scope:

• Work closely with clinical departments to initiate and implement clinical pathways and case management programmes and activities
• Screen all new admissions cases to identify cases for case management 
• Assess patients' medical, psychological, social, and financial status to formulate patient care plans 
• Monitor and evaluate the patient’s response to treatment plan and suggest for revision if needed
• Ensure patients' diagnostic tests, treatments, and consultations take place promptly and provide patient education accordingly 
• Initiate, implement and coordinate the discharge planning process for patients appropriately
• Identify system, practice, patient, and community issues and implement appropriate remedial actions to address deficiencies found
• Provide education, support and training related to case management functions to doctors, nurses and allied health professionals 
• Responsible for the development, implementation and evaluation of clinical pathway.
• Establish a framework for data collection and analysis on care delivery process 
• Prepare statistical reports and participate in projects related to case management functions 

Job Requirements:

• Minimum Degree, preferably in Nursing or Non-registrable MBBS
• Excellent interpersonal and communication skills
• Possess basic statistic skills and analytical skills.
• Proficient in using MS office
