Job Description:

• Assess and identify patient's medical, psychological, social, environmental and financial needs.
• Formulate plans of patient care in collaboration with clinicians, other healthcare professional and patients. 
• Screen all news admissions daily to identify cases for cases management and receive referral from ward. 
• Ensure patients' diagnostic tests, tratments, and consultations take place promptly and in proper sequence. 
• Conduct training  and education on case management for medical team, including doctors, nurses etc. 
• Coordinate and participate in disease management programs. 
• Prepare reports and educate patients if required.

Requirements: 

• Minimum Bachelor of Nursing or Non-registrable MBBS
• Minimum  4-5 years relevant experience.
