Our esteemed client is looking for Case Management Officer.

Responsibilities:

· Responsible for day to day operation such like screening patients in all cardiology clinics for suitability.

• Recruiting patients as well as liaising with care coordinators to follow up on appointments.

• Contacting patients who have defaulted their appointment.

• Counselling patients on the importance of chronic disease.

• Creating database to track patient’s records.

Requirements

· Minimum Bachelor Degree in any Science related Degree

· Service-oriented

· Good Communication Skills
