Case Manager (Youth Department - Integrated Service Provider)

Responsibilities:

• Working with youth at and in risk e.g. youth who are youth offenders.
• Provide case management and appropriate level of care through risk assessment, care planning, and interventions to youths at risk and their families.
• Collaborate with schools and community stakeholders to implement a holistic intervention and supportive care plan for youths and families.
• Conduct and facilitate groupwork sessions according to the curriculum provided to youths.
• Plan and executive community-based programme.
• Contribute to work teams and projects in the Youth Department.

Requirements:

• Passionate in working with youths
• Candidate possesses minimum Degree/postgraduate certification in Psychology, Counselling or Sociology.
• Preferably trained in YLS/CMI (Youth Level of Service/ Case Management Inventory).
• Willing to work one night every fortnight and on some weekends to execute community-based programme.
• Have good interpersonal and administration skills
• Strong team player with excellent organisational skills
• Job position is on contractual basis
