Our Client, a leading and well-known Health Institution in Singapore is inviting qualified candidates to fill the position as Clinic Care Coordinator.

Job Responsibilities

Assist in the care management team to provide basic screening and measurements for chronic patients.
Educate patient on home monitoring devices and use of Healthhub portal.
Assist in planning, organizing and coordinating workplace health promotion activities for staff and perform any other duties as assigned by supervisors.
Make appropriate referrals to care managers, assist patients to schedule follow‐up appointments for consults, screenings and vaccinations in relation to chronic care management/population health.

Job Requirements

Min Nitec and above
A team player with good interpersonal & communication skills
Passionate to work in Healthcare industry
Customer-oriented, a team player, a fast learner and serious with work
