Job Description - Community Care Lead/Coordinator

Roles & Responsibilities

My client is seeking an experienced healthcare professional to lead a team focused on post-discharge care coordination and community integration. This role involves strategic planning, team leadership, and cross-sector collaboration to improve patient outcomes and reduce unnecessary hospital readmissions.

Key Responsibilities:

  • Lead and mentor a team of care coordinators supporting patients and caregivers after discharge
  • Oversee follow-up care through phone outreach and home visits, ensuring patients can safely transition back into the community
  • Coordinate with healthcare partners to streamline services and strengthen support systems
  • Develop and implement frameworks that enhance community care delivery
  • Supervise clinical performance and guide professional development for team members
  • Monitor data quality and contribute to service improvement initiatives
  • Participate in internal projects to improve care standards and operational efficiency

Requirements:

  • Professional background in Nursing, Social Work, Allied Health, or related clinical fields
  • Minimum 8 years of experience in care coordination or community health
  • Proven leadership experience (35 years) managing multidisciplinary teams
  • Strong communication, planning, and stakeholder engagement skills
