Medical Social Worker, Psychological Medicine, REACH (Contract)

This position supports the mental health and well-being of children and adolescents in schools through early assessment, intervention, and clinical social work services.

The role strengthens school and community capacity through collaboration, caregiver support, and multidisciplinary care to ensure holistic outcomes for students and families.

Duties and Responsibilites

Clinical Role

• Conduct bio-psychosocial assessments for referred students.


• Provide psychological support and intervention (individual and/or family therapy) to students and caregivers.


• Monitor students’ progress and review care plans.


• Provide financial assessment and assistance to students who require financial aid for medical treatment.

Case Management Role

• Provide consultation and guidance to schoool counsellors. 


• Develop care management plans with school counsellors and community partners.


• Support right-siting of students for clinic follow-up.


• Coordinate transfer of care and monitor attendance and adherence.

Liaison Role

• Initiate referrals and liaise with healthcare and community agencies.


• Share relevant SSA resources with students, caregivers, and school counsellors.

Other Responsibilities and Duties

• Take on quality improvement projects and activities.


• Contribute to learning and sharing in department case conference/journal club.


• Support other duties as assigned by the department.

Requirements

• Degree in Social Work 


• 2 years of relevant work experience preferred.


• Good communication and interpersonal skills.


• Competent in IT skills.
