Palliative Care Team as a Medical Social Worker

Provide palliative & end-of-life psycho-social support for the elderly and their families

Make financial assessments & casework counselling through home visits team with other professionals to provide a holistic, multi-disciplinary approach to elderly beneficiaries

Liaise with community service organisations and agencies to bring all resources to bear in palliative care.

What will you need to thrive in the role?

A degree in social work or recognised post-graduate diploma in social work from an accredited university.

Be a registered medical social worker with the Singapore Association of Social Workers

Minimum 2 years of relevant palliative and end-of-life care experience or equivalent relevant experience

Excellent interpersonal skills good analytical and interviewing skills

Ability to work independently

Good self-care habits
