Pharmacist - PUBLIC HEALTHCARE / ATTRACTIVE SALARY (HMC) - Immediate Start

• Performed medication reconciliation, to identify medication(s) taken prior to admission, drug related problems leading to current admission and documented it
• Handling stocks and order supplies of medicines in unit of dose to the ward
• Processing medication orders and packing in accordance to SOP
• Responded to queries or drug information to patients, caregivers, healthcare professionals, students and/or public
• Performed medications counselling, inhalers technique counselling, warfarin counselling to the patients and/or caregivers
• Review and process medication orders and provided pharmaceutical care plan to the patient

Requirements:

• Recognized Degree in Pharmacy
• 3 years and above of relevant experience
• Able to perform shift duties
