Job Description - Programme Executive, Reintegration

Purpose of the Role

The Programme Executive plans, delivers, monitors, and evaluates centre-based programmes that support the needs, goals, and well-being of Persons with Disabilities (PWDs). The role involves hands-on programme delivery, day-to-day client engagement and support, and close coordination with caregivers, volunteers, service providers, and community partners. The Programme Executive also manages programme administration, documentation, and reporting, ensuring services are delivered safely, consistently, and in line with organisational standards and intended outcomes.

Responsibilities:

Programme Planning, Delivery, and Quality Improvement 

  • Plan, develop, implement, and review centre-based and community programmes and events in alignment with organisational objectives and client needs.
  • Manage resources to ensure timely, safe, and effective service delivery.
  • Evaluate programmes through internal reviews, client outcome tracking, and satisfaction surveys, in collaboration with Case Management.
  • Collect, analyse, and report programme data and feedback to identify areas for improvement and implement follow-up actions.
  • Provide operational coverage for the Programme Coordinator when required to ensure continuity of services.

Client Management & Case Support

  • Conduct Initial Assessments (IA) and maintain quarterly Individual Care Plans (ICP) in collaboration with Case Management (CM).
  • Monitor, document, and report clients’ participation, progress, and outcomes across programmes.
  • Implement Behavioural Management Plans and individualised interventions, including follow-up and documentation.
  • Serve as the primary point of contact for caregivers on client well-being, including progress updates, healthcare matters, safety, behavioural concerns, incidents, and quarterly progress reports.

Volunteer Coordination & Support

  • Assist with volunteer induction and onboarding for programmes and events.
  • Provide guidance and support to ensure volunteers are effectively integrated into programmes.
  • Manage and resolve volunteer-related issues, including behavioural concerns, in collaboration with relevant staff.

Stakeholder & Community Engagement

  • Build and maintain collaborations with community partners and external organisations.
  • Coordinate programme-related marketing and communications, including promotions, materials updates, and announcements.
  • Conduct outreach and invitations to clients for programmes and community events.

Administrative & Financial Responsibilities

  • Track programme expenditure, process monthly invoices, and assist with budgeting and expenditure monitoring.
  • Support procurement, inventory management, and storage of programme items and medical consumables.
  • Support annual budgeting and programme-level financial planning.
  • Compile and analyse programme data—including attendance, session delivery, and client satisfaction—for reporting and planning.

Healthcare, Safety & Risk Management

  • Advise caregivers and stakeholders on clients’ healthcare matters and follow up as required.
  • Implement and monitor safety measures within programmes.

Staff Coordination & Capability Development

  • Monitor staff competency in programme delivery and client management.
  • Support staff training arrangements and development initiatives.
  • Provide feedback, address performance concerns, and support appraisal processes in collaboration with management.
  • Assist in managing people-related issues and operational challenges within the programme team.

Requirements:

  • Degree in Social Work, Psychology, Special Education, Human Services, or a related field.
  • Experience in programme development, delivery, and evaluation, as well as client management in the social service sector, preferably working with PWDs.
    Strong interpersonal and communication skills.
  • Ability to work effectively with multidisciplinary teams and external partners.
