As a Senior Medical Social Worker, you will be responsible for the following duties:

• Assess the psycho-social status of the patients and families to determine the appropriate care plans.
• Counsel and support the patients and families associated with the illness.
• Work closely with community partners for the welfare of the patients.
• Deliver public education talks.
• Maintain working relationship with other disciplinary teams on patient care plans.
• Work with other provider to provide appropriate services to ensure continuity of care.
• Share the sessions with stakeholders and other professionals.
• Maintain the right documentation for case management and audit purposes.

Requirements:

• Minimum Degree in Social Work
