- Perform psychosocial assessment, intervene and support wellness in the family system.
- Work with multi-disciplinary team in management of cases.
- Lead and plan caregiver support activities.
- Liaise with community stakeholders for case management and/or referrals.
- Supervise and manage medical social workers and admin personnel.
- Initiate and lead projects/services. Take part in giving talks and presentations to internal and external stakeholders.
- Provide any other administrative support to the Head, Social Work as and when required.
- Any other related duties as directed by the said reporting officers.
Qualification
- Degree in Social Work
- Registered with Singapore Association of Social Work
- At least 5 years of relevant working experience in healthcare industry
- Conversant in local dialects and/or Malay, Mandarin, Tamil
- Strong knowledge in sector landscape and community services.
