(Senior) Principal Medical Social Worker

Assist Head of Department in ensuring that the department’s performance indicators are met Assume leadership to projects and portfolios with professional/ organizational implications Provide clinical consultation and support to complex casework requiring intervention at system level. Clinical supervision and mentoring 

Primary Responsibilities and Duties

1) Assume leadership to key projects with professional and organizational implications, eg LOS Task Force 2) Supervision on Casework: to work on case assessment and action plan. Preliminary case assessment is to be completed by 2nd to 3rd day of referral. Complete assessment and action plan is to be completed by 3rd to 5th day of referral. Case progress is to be regularly reviewed until patient is discharged home. For patients who we anticipate requiring continue support after discharge from hospital, post-discharge follow up is required until social situation is managed and stabilized. 3) Supervision on Discharge Planning for continuing care: to provide counselling and support to patients and family members who require special support for care upon discharge from hospital. To provide comprehensive assessment in the psycho-emotional state of patient and family members, their pre-admission coping mechanism, social and financial resources and availability of relevant community resources. To liaise with community to facilitate effective post-discharge support. 4) Supervision on Crisis Intervention: to provide priority response within same day or by next working day, and intensive case management to patients who go through personal crisis including attempted suicide or are victims of abuse. 5) Supervision on Financial Assessment and Assistance to patients who require financial aid for medical treatment. 6) Teamwork with multi-disciplinary team to achieve holistic care for patients 7) Liaison with community organisation for proactive case management.
