Job Description - Senior Social Worker/Case Worker at Mobile Support Team (COMIT)
SAMH manages the Mobile Support Team (MST), it is a community based allied health-led team which provides psycho-social therapeutic interventions for clients with mental health needs and support for their caregivers to maintain clients wellbeing in the community.
Purpose
Responsible for the provision of case management services and support for persons in recovery from mental health conditions/possible mental health conditions, and their caregivers.
Overall job objectives
To be responsible for overseeing the day-to-day administration and case management for COMIT team. To ensure the smooth flow of processes and SOPs are in place to guide and support staff in the delivery of services to clients and their caregivers as well as to meet programme KPIs set by funders.
Core responsibilities
Programme Management
Demonstrate knowledge and understanding of mental health illness, intervention modalities and services to manage mental health needs of clients and caregivers.
To assist in the screening of referrals and follow-up processes, conduct risk assessment and safety planning during the assigning of cases to staff for follow-up with intervention plans.
Takes the lead in facilitating COMIT internal case discussions and facilitating ASCAT case discussion as well as ad hoc case management or clinical projects using the social work framework.
To readily identify and assess risks inherent in moderately complex cases by integrating a range of information for case formulation.
To readily use professional tools and working templates to guide staff in case assessment and case planning.
To conduct regular case conference and case discussions with participating agencies when required.
To oversee the provision of individual and family case management intervention & support for the team as well as to provide guidance and recommendations on the approval of referrals to other agencies required as part of client’s care plans or case management interventions in areas such as:
Employment and work-related matters;
Medication management and coping with side effects;
Budgeting and financial management;
Self-care;
Activities of daily living (ADL);
Home living and independent living skills;
Conflict resolution and mediation;
Interpersonal issues and family reconciliation;
Discharge planning and accommodation issues.
To network and liaise with employers, psychiatrists, social workers, caregivers and other helping professionals and agencies for a holistic case management approach.
To oversee the management of behavioral and mental health concerns presented by clients.
To provide clinical supervision and guidance to social work interns and provides performance evaluation.
To assess clients for readiness to live independently in the community, to oversee the sourcing of alternative accommodation in the community and resources to furnish their new accommodation.
To engage the police for help to handle crisis situations and violent behavior where necessary.
Perform the necessary screening tools for clients’ assessment and Client Satisfaction Survey.
To establish positive networking relationships and collaborations with community partners and funders.
To help Deputy Head in the clinical supervision of ground staff and monitor their compliance to professional processes (such as the use of tools and templates, adherence to practice policies).
To help Deputy Head assess the ground staff capabilities, identify their professional growth areas and build training interventions to meet the growth areas.
Staff Management
Demonstrate team leadership and be a role model in guiding team members in their professional conduct, integrity and ethical boundaries.
Has people development competencies to translate own knowledge and skills to contribute to developing staff’s learning and growth and is able to support team members in managing challenging/high risks cases.
To provide guidance in identifying early symptoms of relapse, provide feedback to the psychiatrists and assist in the arrangements for clients’ admission to hospitals for in-patient treatment; educate clients to advocate for their own recovery by working in collaboration with the medical team.
Support the Deputy Head in leading and guiding the team in carrying out their responsibilities such as ensuring timely submission of case notes, reports and statistics and ensuring the smooth running of workgroups and activities etc.
To be responsible for updating Deputy Head on the team’s movements, calendar updates and attendance.
Administrative Support
To oversee the writing of case reports, case recordings and letters to agencies on behalf of clients by the team.
To oversee the compilation of updated records of clients’ particulars and statistics for monthly reports and to meet funders’ statistical requirements.
To oversee the assessment of clients’ eligibility and to approve the application of external services and referrals where necessary.
To oversee the taking of minutes during case conferences and staff meetings.
Outreach Involvement
To orientate and host visitors to SAMH centres.
To conduct in-house and external talks and participate in events to promote SAMH services and awareness of mental health concerns.
To sit in work-related committees in collaboration with other community agencies.
Management Competencies
Demonstrates strong communication skills with different stakeholders and has the ability to bring out the best in others. Is an effective bridge between management and staff with the ability to establish positive networking relationships and collaborations with community partners and funders.
Demonstrates delegation & control competencies by recommending the acceptance or rejection of new referrals screened before assigning accepted cases to staff in the respective teams.
Demonstrates analytical & decision-making abilities to manage meetings more effectively so that high value conversations take place with the ability to exercise considerations from a management’s perspective.
Demonstrates planning & strategic thinking which is aligned with social work values such as service to humanity, social justice, dignity and worth of the person as well as the importance of human relationships.
Demonstrates having the innovation to contribute to the continuing success of the organisation by supporting the Programme Head to manage, supervise and ensure the smooth operations of Case Management Team.
Demonstrates strategic planning when calibrating the caseload managed by staff and to recommend for the closing of cases.
Other Responsibilities/Duties
To lead staff or participate in ad hoc research or other projects when required.
To collaborate with and support other SAMH programmes or projects when required.
Assist in coverage of outreach duties when required.
To be on stand-by to provide support for team when alerted in an emergency to deal with sudden crises.
Qualifications
At least a degree in Social Work or related subjects.
At least five years of direct work experience in direct case management. Experience in mental health work would be an advantage.
Demonstrate ability to handle work professionally, independently and efficiently with clear ethical considerations.
Possess effective organizational and follow-up skills, paying attention to details of programme’s needs and trends.
Good interpersonal skills and able to interface with all levels, internally and externally, in a professional and responsive manner.
To be self-motivated and self-directed but yet to follow directives and programme guidelines. Have good knowledge of social work practices, counselling, case management, group work facilitation, preferably some knowledge of mental health.
Possess a pleasant disposition to deliver services using the Psychiatric Rehabilitation approach.
Efficient and professional services are delivered by the team to clients.
Case management processes are in place to support clients’ needs from admission to discharge.
Service KPIs are met and submissions to funders and for in-house needs are timely and accurate.
