Social Work Associate

The Social Work Associate supports the Medical Social Work team in providing psychosocial care to patients and caregivers who is known in St Luke’s Outpatient Clinic (SLCC) and Home Care Services (HCS).  Working under the supervision of a Medical Social Worker (MSW), the Social Work Associate provides support in case coordination, financial assistance administration, programme coordination and administrative functions.

The Social Work Associate will provide support in the following areas:

· Assist with the screening and triaging of referrals.

· Support care coordination and casework interventions under the supervision of a Medical Social Worker (MSW).

· Liaise with patients, caregivers, the multidisciplinary team and community partners to facilitate service coordination.

· Attendto enquiries relating to financial assistance schemes, community resources and long-term care services.

· Assist with applications for financial assistance schemes, grants and subsidies under the supervision of an MSW (e.g. Medi Fund/Medi Fund Silver, AIC Long-Term Care schemes, Seniors' Mobility and Enabling Fund, SG Enable Assistive Technology Fund, and other relevant assistance schemes).

· Assist in organising and coordinating caregiver support programmes, group work and other departmental activities.

· Perform data entry and maintain accurate records.

· Assist in the preparation of statistics, reports and service outcome data.

· Provide administrative and secretariat support, including coordinating meetings, taking minutes and supporting committee work.

Job Requirements:

· Possess minimum a Diploma in Social Work or WSQ Diploma in Social Services

· Experience in the healthcare or Social Service Agencies will be an advantage

· Proficient case coordination skills to support Medical Social Work interventions

· Good knowledge of community resources

· Team player with good interpersonal and communication skills

· Proficient in Microsoft Office applications
