Job Description - Social Work Associate, Transitional Shelter

RESPONSIBILITIES
Allkin's Transitional Shelter (TS) provides temporary accommodation to homeless individuals and families who have exhausted all other means of accommodation and require social work interventions.
Roles & Responsibilities:
Clinical & Programme Related
Process referrals by conducting eligibility assessments and screening applicants for shelter admission.
Assist Social Workers with shelter visits, direct client assessments, and the documentation/review of intervention strategies.
Follow up and monitor post-discharge status and outcomes of clients and liaise with agency on progress of
Plan and implement therapeutic or developmental group programs for residents
Arrange case conferences with referring agencies to identify client needs prior to admission.
Assist with data compilation and Excel documentation submissions.
Operations
Serve as point of contact for resident inquiries and support newly admitted clients through pre-admission conditions.
Manage shelter financial matters and coordinate unit turnovers for new admissions or discharges.
Conduct routine unit checks to enforce safety protocols and mediate interpersonal conflicts between residents
QUALIFICATIONS
Preferred Diploma in Social Work; other diplomas will also be considered.
Comfortable traveling between work locations and occasionally working at different work hours.
Capacity to respond effectively to shelter emergencies and work calmly with clients in complex, high-stress situations
Strong teamwork, organizational and documentation skills, with proficiency in MS Office (specifically Word and Excel) for accurate case recording
Proficient verbal and written communication skills.
