Job Summary

You will conduct casework, counselling, and home visits to support residents. Advise on social assistance schemes, assess referrals, develop care plans, and collaborate with agencies to deliver community programs and manage volunteers.

Responsibilities

• Conduct casework, counselling, and home visits to support residents’ social and welfare needs
• Advise and assist residents in accessing social assistance schemes while tracking their progress and development
• Analyze community needs and implement targeted programs to address those needs effectively
• Assess all referral cases for admission based on established criteria and follow up on renewals
• Develop individualized care plans tailored to each client’s requirements
• Assist to identify clients with early onset of disease or conditions such as depression, dementia.
• Collaborate and network with relevant agencies to enhance client welfare, volunteer management, and program delivery
• Befriend & provide mental or social support to residents, caregivers, families.
• Guide and supervise volunteers/interns assigned to social work tasks and community activities
• Participate in and assist with community events and activities to support outreach and engagement efforts
