Responsibilities
Reporting to the Head of Department, the incumbent will be responsible for the following:
1. Manage and monitor ongoing cases of residents and day care clients through comprehensive psychosocial assessment, intervention planning, and case management.
2. Prepare, implement, and regularly review Individual Care Plans (ICPs) in collaboration with residents, caregivers, and multidisciplinary team members
3. Maintain accurate and timely documentation in the Care Management System (CMS), including case notes, assessments, reports, and submissions to agencies such as MSF and SGEnable, including EPES and Case Central Management requirements where applicable.
4. Present cases to the Admission & Review Committee for assessment, recommendation, and approval.
5. Participate actively in multidisciplinary case discussions with Nursing, Therapy, and Allied Health professionals to ensure coordinated and holistic interventions.
6. Facilitate and coordinate Means Testing renewals for residents to ensure continuity of subsidies and financial assistance.
7. Explore and facilitate community integration, vocational training, employment opportunities, and external placements for beneficiaries who demonstrate readiness and interest.
8. Contribute to programme evaluation, research initiatives, and development of new programmes and services within the organization (if any).
9. Perform any other duties assigned by the Supervisor or Management.
Requirements
1. Registered Social Worker with a Bachelor’s degree or Graduate Diploma in Social Work.
2. Minimum 4 years of social work experience in a Social Service Agency, Ministry of Health setting, nursing home, or a similar capacity.
3. Training in suicide intervention or the management of violence and abuse would be an advantage.
4. Experience working with persons with disabilities or elderly clients would be preferred.
5. Strong communication and interpersonal skills.
