Job Description - Social Worker / Case Worker (TOUCH Development &Therapy Services)
Job Summary: The incumbent is responsible for delivering holistic case management services for children and their families through engagement, assessment, intervention, and follow-up. This includes conducting bio-psycho-social assessments, developing and implementing care plans, and working closely with caregivers, educators, and community partners to support the child’s development. The role also ensures accurate documentation, effective communication, and coordination of resources to achieve positive outcomes for children and families.
Principal Responsibilities and Duties: 1. Engagement and assessment
Engages in evidence-based and culturally sensitive practices when facilitating family sessions or community engagement sessions.
Collates and consolidates all information about the child and family from relevant professionals and sources in the school.
Establishes rapport, affirms parents and uses various strategies to engage parents.
Facilitates parents’ understanding of programme, profile of child in different settings and triangulates information with recommendation(s) from the team.
Conducts holistic bio-psycho-social assessment of child’s and family’s needs, risks and strengths, for intervention.
Facilitates all documentation needed for enrolment in programme and provides relevant resources for family/caregivers.
Communicates effectively to parents in verbal and/or written form
2. Goal Setting and Intervention
Participates in case discussion by providing information about child and family strengths and needs and identifies possible care plans required for the child and/or family.
Engages and works collaboratively with various community agencies (e.g., Family Service Centres, Social Services Offices, or other agencies) to ensure holistic case management for families.
Develops appropriate case conceptualisation and intervention plans based on relevant theories.
Collaborates with all caregivers/educators of child (including parents/caregivers, EC educators and DS LS Professionals) to support child and family development (e.g., organising and/or participating in case discussions with multi-disciplinary team).
3. Evaluation and Documentation
Completes documentation (e.g., case notes) accurately and communicates information effectively to relevant professionals.
Evaluates and identifies areas required for follow-up (e.g., referral to appropriate internal and/or external resources)
Evaluates goals and intervention plans required for follow-up/ reinforcement at home, in school or in support sessions and modifies goals and plans appropriately in consultation with parents and other relevant professionals.
Handle administrative duties including finance, coordinating with ECDA, and fulfilling all SSNet ES duties", which I think can be added under Evaluation and Documentation
4. Collaborative Partnerships and Community Network
Demonstrates knowledge of available community resources.
Utilises appropriate community resources to support child and/or family.
Networks with all relevant stakeholders and partners (e.g., at local and/or regional level) for collaborative problem-solving
Essential Skills and Qualifications:
At least a relevant diploma in social work, social service or early childhood
A minimum of 2 years of experience within the community or pre-school landscape, with an understanding of working with families with young children
Effective communication skills (verbal and written) to engage children, families, and stakeholders
Case management and care planning skills, with the ability to coordinate and follow through on interventions
Interpersonal skills with the ability to build rapport and work with diverse and vulnerable families
Stakeholder management and coordination skills across multi-disciplinary teams and community partners
