Social Worker (CREST)

Job Summary:
As a Social Worker, you will be responsible for organizing and conducting outreach to increase mental health and dementia awareness and promote early identification. You will support clients who are at risk of mental health issues and/or dementia by linking them to the necessary support and resources. You will continue to monitor these clients in the community.

You will also be involved in data collection and evaluation of the CREST programme.

Principal Responsibilities and Duties:
1. Programme Development and Implementation

• Provide care coordination and service linkage to clients with or who are at risk of mental health and/or dementia, with the appropriate resources and support including caregiving support.
• Plan and conduct outreach to the community and stakeholders including faith-based, corporate and community organisations to promote mental wellness and mental health awareness. 
• Plan and conduct targeted mental health and dementia case finding/screening to promote early identification and facilitate timely support. 
• Plan and organize structured preventive activities and/or programmes for the community, those at risk of mental health conditions and clients. 

2. Administration and Staff Development

• Responsible for maintaining client records that are accurate and timely for submission to AIC for data analysis, in accordance with the CREST programme requirement. 
• Keep and maintain client and programme data and documents, in accordance with the CREST programme requirement, as well as the prevailing personal data protection and security guidelines. 
• Support in the development of CREST service planning document and manual including set up requirements (infrastructure, manpower, training, recruitment) and processes (outreach, screening, mental health information, service linkage & follow-up). 
• Ensure attendance in training and supervision sessions, where relevant, as part of the staff competency required by the CREST programme.  
• Participate in relevant meetings with key stakeholders including AIC to share key learning points where required. 

Essential Skills and Qualifications:

• Bachelor's Degree or Postgraduate Diploma in Social Work recognised by the Singapore Association of Social Workers (SASW).
• Registered Social Worker (RSW) with SASW is preferred or eligible for registration.
• Relevant experience in mental health, dementia care, community-based social work or healthcare setting is preferred.
• Experience in case management, psychosocial assessment and community outreach will be an advantage.
