RESPONSIBILITIES

As a Social Worker in the Senior Service team, you will play a vital role in supporting seniors to age well within their communities. You'll walk alongside them and their caregivers to strengthen their well-being, preserve dignity, and enable ageing-in-place. Through meaningful engagement, coordinated care, and thoughtful casework, you will be part of a dedicated team shaping a more inclusive and elder-friendly society.

Roles & Responsibilities

• Conduct triage and intake assessments for seniors referred to our services.
• Provide case management for individuals and families, with a focus on person-centred care planning.
• Conduct home visits to better understand the senior's environment and develop holistic care plans.
• Perform psycho-social assessments to identify risks and strengths and recommend appropriate interventions.
• Work closely with caregivers, healthcare professionals, and community partners to coordinate seamless care and support.
• Ensure timely and accurate case documentation and reporting.
• Contribute to internal projects, reports, and process improvements when needed.
• Participate in team meetings, supervision, and ongoing training for continuous growth.

• Bachelor's, Postgraduate Diploma, or Master's in Social Work.
• Open to new graduates and early-career professionals in social work.
• Genuine passion for working with seniors and supporting caregivers in a community setting.
• Able to work independently while contributing to a collaborative team environment.
• Strong interpersonal, communication, and organisational skills.
• Proficient in written and spoken English.
• Fluency in a local Mother Tongue language and/or dialects is a plus.
