Social Worker, Seniors Services (contract till Dec 2026)

JOB PURPOSE

This role focus on developing a place-based model of care and building partnerships in local neighbourhoods for the betterment of the seniors. Scope of work include coordinating social health services and connecting seniors to relevant service providers to enable them to better manage their health or illnesses. Provide case management service for vulnerable seniors and their caregivers who face difficulties in managing their multiple health and social care needs.

KEY REPONSIBLITIES

• Work with leadership team of the partner organisations to align goals, establish roles, and develop strategies and workplan for the sub-region to achieve the deliverables specified for Phase 1 and 2.
• Build and develop linkages and protocols between different community providers to manage clients across multiple social-health services
• Work with ICCP providers to trial joint referral workflow in Phase 1 to support care coordination deliverables in Phase 2.
• Work with the AIC’s sub-regional Relationship Manager on the reporting and monitoring of indicators.
• Conduct intake and triage services for members of the community
• Provide casework, counselling and service coordination for seniors and caregivers
• Document and review case, interventions and outcomes within set timelines
• Liaise and work with organizations, partners and volunteers to build an ecosystem of assets and resources to facilitate more effective work with seniors and caregivers.

QUALIFICATIONS

Bachelor/Graduate Diploma in Social Work or relevant fields (Psychology, Community Work)

REQUIREMENTS

• At least 2-3 years of relevant working experience with good knowledge in eldercare or healthcare setting would be advantageous
• High proficiency in written and spoken English with good facilitation and presentation skills
• Strong interpersonal and communication abilities with individuals and systems at multi-levels
• Team player with an ability to function effectively in a fast-paced environment.
• Strong organization skills and structured.
